ACL Tear: Is Surgery Necessary?

With an ACL tear, what decides the outcome is the rehabilitation that follows, as much as the tear itself.

The anterior cruciate ligament (ACL) is the main ligament that controls the knee's rotation and forward sliding; it usually tears during sudden change-of-direction movements. The most common question: is surgery necessary?

Not every tear needs surgery

The decision is made based on age, activity level, additional injuries in the knee (meniscus, other ligaments), and whether there's a sense of the knee "giving way." Surgery is often chosen for young athletes wanting to return to sports with sudden changes of direction; for someone aiming for a less demanding life, a strong rehabilitation programme alone may be enough. The decision belongs to the orthopaedic surgeon.

Surgery or not: rehabilitation is essential

On either path, the muscles around the knee need to strengthen and control needs to return. Even if the surgical decision is made, the person doesn't go straight to the table: a preparation (prehab) period is applied until swelling goes down and movement and muscle control return — going into surgery with a strong knee noticeably eases what follows.

Expectation

Recovery spans months and is measurement-based. The knee "not hurting" isn't enough — returning to demanding activity before strength and control are adequate is the most common cause of re-injury.

Is the other leg also at risk?

Yes, people who've had an ACL tear in one knee can have an increased risk of re-injury in the other knee too; this is usually because the strength and control imbalance between the two legs was never fully resolved. That's why rehabilitation covers both legs, not just the operated side.

When to see a physician?

The knee locking, unbearable pain, noticeable swelling, or a sense of giving way call for orthopaedic assessment.
