Don't let "flattening" on the report scare you

Neck flattening isn't a disease on its own. It's also seen on the scans of people with no pain at all, and it's often a result, not a cause: in a painful neck, muscles contract protectively and the curve temporarily reduces. In many people it comes back on its own once the pain is gone.

How important is the straightening on the film?

The honest answer is: less than it is assumed to be. A reduced neck curve can be the result of protective muscle guarding in a painful neck — that is, a consequence rather than a cause. When the pain settles, the curve returns on its own in many people.

Knowing that changes the treatment: the aim is not to correct a line on an X-ray but to restore the neck's capacity and mobility. The curve returning to normal is usually a by-product of that, not the target itself.

What really decides it: the deep neck muscles

Small muscles lying deep at the front of the neck provide the fine control in carrying the head. When they weaken or lose endurance, the large surface muscles take the job over — and they stay contracted all day, producing pain by evening.

Restoring the endurance of these muscles sits at the centre of the programme. It is not a strength exercise; done correctly it is almost invisible, yet it is one of the best-evidenced approaches in neck pain.

The neck is not alone: the upper back's share

When the upper back loses mobility, the movement the trunk needs is transferred to the neck. The neck has to carry that extra load and ends up working constantly at the edge of its capacity. This is why a programme aimed only at the neck makes limited difference for most people.

The assessment therefore covers the upper back and shoulder girdle. Once scapular control is in place, the load on the neck falls even if the same time is spent at the same screen.

When to see a doctor

A straightened cervical spine is not an emergency in itself. However, numbness and weakness spreading into the arm, loss of hand dexterity, unsteadiness when walking, unexplained weight loss, fever, or neck pain starting after trauma all require medical assessment.

Frequently asked questions

Is a straightened neck dangerous?

It is not a dangerous finding on its own and is also seen in people without pain. What matters is not the image on the film but which symptoms you actually have; the programme is built around those.

Will the curve come back?

In some people the curve improves on its own as pain and muscle tension reduce. Success is not measured by that, though; the aim is less pain and a neck that is comfortable to use in daily life.

Should I buy a special pillow?

Pillows marketed for a straightened neck have not been shown to be superior. What matters is not the type but that the height suits your sleeping position, which is decided together at the assessment.

Are neck stretches enough?

Stretching relieves symptoms in the moment but does not create lasting change on its own, because it does not alter why the muscles stay tight. Lasting results come from deep neck muscle endurance and upper back mobility.
