Pain spreading to the shoulder and arm with numbness often comes not from the shoulder itself but from the neck. How to tell them apart, what helps, and when to see a doctor.
What people search as "nerve compression in the shoulder" often comes not from the shoulder itself but from the neck: when a nerve root in the neck is irritated, pain can spread to the shoulder, shoulder blade and arm. Separating this from a true shoulder problem (impingement, tendon, frozen shoulder) decides the direction of treatment from the start — because shoulder exercises do not help pain that comes from the neck.
| Clue | More likely from the neck | More likely from the shoulder |
|---|---|---|
| Spread of pain | From the neck to shoulder, arm, fingers | Stays around the shoulder |
| Numbness-tingling | Often in the arm and fingers | Usually none |
| What changes it | Neck movement, certain head positions | Arm movement, raising the arm |
| Raising the arm | Usually possible but the neck is uncomfortable | Catches/hurts at a certain angle |
| At night | Neck position matters more than arm position | Worse lying on the painful shoulder |
This table is guidance, not a diagnosis; more than one cause can be present together.
Less commonly, some nerves in the shoulder-neck region (for example the nerve supplying a shoulder-blade muscle) can be irritated along their own path, causing pain at the back-top of the shoulder and weakness in certain muscles. Still, the most common source of pain that spreads to the shoulder/arm with numbness is the neck. For the neck side, how neck hernia settles and how a stiff neck settles can guide you.
It depends on the source, but common themes:
Progressive weakness in the arm, loss of hand skill (buttoning, dropping objects), numbness in both arms at once, changes in bladder-bowel control, fever, or severe pain that does not settle at rest need prompt medical attention. In sudden, severe, unexplained shoulder-arm pain, other causes (e.g. the heart) must be ruled out.
The assessment separates whether the pain comes from the neck or the shoulder; neck movements, arm tests and nerve tension are assessed together. The programme is built on the source: targeting the neck if neck-related, the shoulder if shoulder-related. For the shoulder side see shoulder pain.
What is the most common cause of numbness spreading to the shoulder and arm? Most often irritation of a nerve root in the neck; that is why shoulder exercise alone is usually not enough and the neck is addressed too.
Can I tell myself whether it is the shoulder or the neck? The clues above give an idea, but the clear distinction is made by examination; exercising for the wrong source wastes time.
